CSM REGISTRATION CHANGE AND REFUND POLICY
- Entry fees are not refundable.
- A $25 administrative fee will be required for any changes requested to your CSM registration.
- Full refunds for accommodation, transportation and banquet will be provided if notice is received in writing at the CSM office before December 1st, 2007.
- Therefore, there will be no refunds for accommodations including Château reservations, transportation and banquet after this date.
To download the registration form, you will need Adobe Acrobat. Please click on the  to begin the downloading the free software if it is not already installed on your computer.
- Fill out the registration form, sign and fax it to the CSM office or send to the CSM mailing address with your cheque or credit card details. Separate registration forms required for each participant (photocopy additionnal copies of the form if necessary).
- If you are staying at Fairmont Le Château Montebello, all roommates must be listed on each form and all roommate registration form must be sent together. A full payment for the room guarantees your reservation.
